Completed Bureau Copies of Form #5 may be mailed to the Lebanon County EIT Bureau, 547 S. 10th Street, Lebanon, PA 17042. Be sure to include your W-2 forms and all applicable schedules.

2007 Local Earned Income Tax Rates
Estimated Tax Payments:
To make an estimated local tax payment for 2007, please complete Form#5 and write the word “ESTIMATED” across the top of the form. Please enclose your check or money order for the estimated tax you have calculated and indicate on your check “2007 Estimated Tax”.
